Continuous Glucose Monitoring System Mechanisms
Continuous Glucose Monitoring (CGM) systems provide real-time data on glucose levels in the interstitial fluid. Unlike traditional finger-stick tests that provide a single point in time, CGM offers a detailed view of glucose trends and fluctuations. This technology is vital for individuals managing insulin-dependent diabetes.
A CGM system consists of a small sensor inserted under the skin, a transmitter, and a receiver or smartphone app. The sensor continuously measures glucose levels and sends the data wirelessly. This allows for the identification of patterns, such as nocturnal hypoglycemia or postprandial spikes, that might otherwise be missed.
The sensor uses the enzyme glucose oxidase to react with glucose in the interstitial fluid, generating a small electrical current proportional to the glucose concentration. It is important to note that there is a slight "lag time" of 5 to 15 minutes between blood glucose levels and interstitial glucose levels. However, for most daily management tasks, the trend arrows provided by CGM systems—which indicate whether glucose is rising, falling, or stable—are more valuable than a static number.
One of the most significant clinical advantages of CGM is the ability to set alerts for high and low glucose thresholds. This provides a safety net, particularly during sleep or exercise, when a patient might not be aware of a dropping glucose level. When integrated with an insulin pump, CGM data can be used to automatically adjust insulin delivery in what is known as a "closed-loop" or "artificial pancreas" system. This significantly reduces the burden of self-care.
The use of CGM has been shown to improve Hemoglobin A1c levels and increase "time in range," the percentage of the day spent within a healthy glucose window. As sensors become more accurate and require fewer calibrations, they are becoming the standard of care for both Type 1 and many Type 2 diabetes patients. The data generated also allows clinicians to have more meaningful discussions with patients about the impact of diet and activity on their glycemic control.
